SCCA Minor Waivers Everyone at an SCCA event must sign a release and waiver form. If you are under 18 years old, or are bringing someone under the age of 18, a special Minor Waiver must be used. The form is fairly simple, but certain procedures need to be followed. Both parents/guardians must sign the Minor Waiver. It is not acceptable to have anyone other than the minor’s
parent(s)/legal guardian(s) execute any waiver on behalf of the minor. Friends, relatives, brothers/sisters etc. may not
sign the waiver on behalf of the minor. If a parent/legal guardian has sole custody or legal guardianship status, please initial the appropriate box on the form. 1. Forms may be obtained at the event site or by going to link listed below. If you use the form below, please print 2 full color copies http://www.scca.com/documents/Club Forms/MinorReleaseFormMS-L_W_Program.pdf
FOR SOLO or RALLY EVENTSMinor waiver forms may be executed by the parents/legal guardians away from the event, but signing at the event, having an SCCA member witness the signing, or having the form notarized is preferred. However, the forms must be “received” by an adult SCCA member at the event site. Please present the forms to the event registrar. FOR CLUB RACING The Parent’s/Guardians signatures MUST be witnessed by an SCCA Registrar or the waiver form MUST be notarized. 3. The Registrar will return one copy of the form to you. You must bring this copy to all subsequent events to show that the Minor Waiver has been completed. These forms are good for one calendar year. |
